Advance care planning (ACP) begins with a discussion to clarify and note personal preferences regarding future medical care. It serves as a guide to loved ones and healthcare providers to help with treatment decisions in the event of a medical crisis.
Join us for the 13th instalment of our Wellness series with Shawn Yon, our Medical Social Worker, as he addresses the role and benefits of advance care planning in improving quality of care and patient outcomes.
Shawn Yon is a registered social worker and a qualified advance care planning facilitator. He obtained his Bachelor of Social Work from the Singapore University of Social Sciences (SUSS) under the Professional Conversion Programme for Social Workers (PCP SW). He has worked in various settings from acute hospitals, social service organisations to intermediate and long-term care (ILTC) settings.
At Allium Care Suites, Shawn works closely with our multi-disciplinary team of doctors, nurses and therapists to provide psychosocial support for residents and their families, including psychosocial and financial assessment, casework and counselling.
