Allium Healthcare welcomes you to a Health Talk and Screening Session facilitated by Medical Director, Dr Jason Tan. Dr Jason Tan will share health tips to prevent and manage the 2 chronic conditions that are more common in older adults; diabetes and hypertension.
Our nurse and therapists will be happy to conduct the following screenings:
- -mini-cog©
- – screening for cognitive impairment in older adults
- -fall risk assessment
- -blood pressure measurement
Dr Jason Tan is the Medical Director of Salus Nursing Pte. Ltd. Trained in Geriatric Medicine, he hopes to provide seamless care to the older adults in the community.
